We are the only independent foundation dedicated to raising funds for palliative care across our region. Our community-based, region-wide mandate means that we are not tied to any particular service, care provider, government or other funding agency, letting us provide support where it is needed most. For the first time in our community, a regional plan outlining improvements in palliative care has been approved. Together, experts, caregivers and service providers from across our region have set out a vision for improvements - a vision that would see palliative care and support available to those who need it, when they need it most. With a plan for change and a commitment to make it happen, momentum is growing. However, there remains a great need for additional resources. This is where the OutCare Foundation can help. We have identified four priority areas where we know additional funding will have an impact on the availability and quality of palliative care in our region, namely: - Providing Care When and Where It is Needed; - Supporting Families, Caregivers and Volunteers; - Connecting People to Information; and - Expanding Knowledge through Education. The OutCare Foundation is dedicated to helping organizations in the palliative care network. All patients and families should receive the care they need, in the setting of their choice for their important end-of-life journey. Canadian Climate Forum The Canadian Climate Forum translates evidence and innovative practices into options for policy, programmes and tools to benefit Canada’s economy and its citizens. Building on over a decade of targeted investment in Canadian climate science through the Canadian Foundation for Climate and Atmospheric Sciences ( CFCAS ), the Forum fills growing gaps in Canada’s weather and climate information that are critical to our health and well-being. Taking a comprehensive and pragmatic approach, the Climate Forum brings together the results of work in natural, social and health sciences to develop options for policy. Its analyses of scientific results and their applications will help inform decision makers on issues from economic development to adaptation to changing conditions. The Canadian Climate Forum organises and supports knowledge translation, studies, workshops and symposia, and outreach activities related to climate systems and atmospheric sciences, and on impacts of and adaptations to changing climatic conditions. It collaborates with all levels of government, policy-makers, the scientific community, the private sector - and international bodies also committed to climate research and policy. The Forum strengthens Canada’s capacity to adapt, mitigate and identify new business opportunities, while reinforcing our intellectual capacity through stimulating work in areas of strategic priority. The Climate Forum’s ultimate goal is a stronger, healthier and more resilient Canadian society. The Forum maintains a small secretariat in Ottawa and is actively seeking funding partners to develop and expand its activities.
