Planning To ensure that our programs and services are accurately reflecting our mandate and responding effectively to community needs, we regularly assess the current and future needs of our community, participate in internal and external planning sessions and carry out research relevant to the needs of our clients. Quality Improvement Plan - in the spring of 2013 we submitted our plan for quality improvement to Health Quality Ontario. This plan builds on the current initiatives of of the Quality Improvement team and outlines our priorities for the coming year. Community Needs Assessment - in 2005/2006, we undertook a community needs assessment to identify and describe the current and future demands for primary health services in Guelph. The assessment helped us to identify options for ensuring that Guelph CHC is able to achieve its mandate within the context of growing demands for services. Balanced Scorecard - in 2006, Guelph CHC underwent a Balanced Scorecard process. This process required us to develop specific performance measures to link the performance of our programs with our overall strategic goals as defined by our Ends Statements . Our staff teams developed objectives, measures, targets and initiatives to outline how each of their programs would honour and link to our strategic goals in an effort to ensure organizational alignment with those goals. Our research projects increase our knowledge base and contribute to our ability to effectively serve our community.
