At-Home Hospice (AHH) is a specialized nursing agency serving the Ottawa Valley. Our mission is to provide compassionate home-based health care to individuals and their families as they face end-of-life. With the growing awareness of the Hospice movement, patients and their families are choosing to stay at home. We have delivered services to thousands of families in the Ottawa area, enhancing quality of life for both clients and their families. We have held a CCAC contract since 2009 and have been subcontracted by other health care agencies in the community. We are a recognized service provider with Veterans Affairs and the Police Force. We also have worked with several insurance companies and legal firms for direct billing and reimbursements. Our RN’s and RPN’s provide care to the pediatric population that remain at home but have special challenges that need professional care. Our staff supports the client and their families while at home or when the child is at school. We have experience with CPAP, BiPAP and home ventilators. We are familiar with various tracheostomy tubes and suctioning methods. As well as G-Tube or PEG Tube feedings. We have cared for several clients with brain injuries, both ABI and TBI and varying in ages from infancy to the elderly. We have learned from each one of the clients and have become more knowledgeable from cares provided. We can provide a detailed Care Plan tailored to the specific needs of the client, and ensure the right match between client requirements and our staff. In order to be able to deliver excellent qualities of care, we hand pick our caregivers and strive to recognize their expertise to enable us to make a good client/staff match. Our focus is to support rehabilitation, reintegration to the community and provide pain and symptom management. AHHC provides services for the frail elderly in the community whether it is in their private homes or retirement residences. All levels of service providers are trained in the care and management of this delicate population. Staff work with allied professionals to rehabilitate clients whenever possible or keep them safe and comfortable in their own home.
