Employs an interdisciplinary team of health professionals to provide primary health care, illness prevention and health promotion programs and services that promote wellness in the community.
Is funded by the Ontario Ministry of Health, through regional health corporations called Local Health Integration Networks (LHINs) - not through OHIP - to provide services to residents of a geographically defined catchment or services area.
Directs most of its programs and services to priority populations - groups of people who face barriers to health and barriers to accessing health care.
