The Elliot Lake Family Health Team vision is a coordinated, responsive and efficient system of healthcare improving the well-being of the people of Elliot Lake and surrounding communities.
Our mission is to work with local partners to advance the health status of our extended community by providing timely access to quality healthcare.
Company description
The overall composition of our Elliot Lake Family Health Team (ELFHT) is key to our ability to better serve and to provide quality health care to the residents of Elliot Lake and surrounding area.
The organizational chart provided outlines the current structure of the ELFHT. The team is staffed with 9 physicians, a complement of 19 full-time positions and 16 part-time positions.
The Elliot Lake Family Health Team first opened its doors July 02, 2007 thanks to the hard work and progressive thinking of a group of physicians at three (3) established clinics (Family Health Centre, Mary Walk and Hillside) and city leaders. They recognized the value of combining resources to help them better manage the shortage of physicians and the challenges of managing many patients with multiple chronic diseases.
This is a new approach to primary care and is best described in a recently published research article in the Annals of Family Medicine.
